OCCUPATIONAL HEALTH and SAFETY PROGRAM
These files are regulated by Occupational Health and Safety Program (OHSP). They document employees on campus with indirect or direct exposure with animals, pathogens, or both. OHSP is in place to ensure that workers are aware of any potential health risks of the job as a preventative safety measure for the employee.

ITEM NUMBER
These files include, but not limited to, the "Hazard Evaluation Form" or HEF, along with the HEF supporting documents, such as; printed email correspondence with the individual, their supervisor, and the Occupational Health staff with the purpose of showing the completion status of the HEF.
Authorized Retention:
Retain files for thirty (30) years after termination of employee; then destroy.
